Scenario:
You are the primary nurse caring for a 14-year-old male receiving treatment for Acute Lymphocytic Leukemia (ALL). Before this diagnosis, he had no other medical issues. He has been undergoing chemotherapy via an implanted access port. The medical center where he is receiving treatment is geographically distant from his home. His mother is a single parent with limited transportation resources and an unstable housing situation. She recently secured employment at a warehouse. The patient’s father is not involved in his care, and there are limited extended family supports. He has responded well to the initial rounds of chemotherapy but has suffered from depression and anxiety related to his diagnosis, infrequent maternal visitation, absence from school, and loss of peer interaction. He is expected to be discharged within the next week but will need to return for chemotherapy treatments every few weeks for the next several months. Due to his treatments, he is immunosuppressed and requires precautions to be undertaken to decrease the chance of infection. You have been involved as a member of the multidisciplinary team during his acute hospitalization to coordinate care. Still, given the social and geographical variables, it has been challenging for the nursing staff to address many elements of the discharge teaching plan.
