Assignment Description:

In this week’s assignment you will be designing a specific EBP question related to addressing a selected Social Determinant of Health (SDoH).

In order to construct a question worth asking, you must first be familiar with what a SDoH is, and what interventions may be appropriate to address one. As our patients have entire lives before admission and after discharge, it is important to understand what their lives look like outside of healthcare facilities. There are many factors in their lives and their communities that often determine how sustainable their health access and outcomes are. While knowing about these determinants is important, knowledge alone is not enough. We, as healthcare professionals, can help address these determinants in order to decrease adverse outcomes and health disparities in our community.

So, once you have identified a social determinant of health that adversely affects your community, you will begin to think of interventions that may be able to address that determinant. You will select a single intervention that could be done, and will transform your idea into a full evidence-based practice PICO question. For the intervention you don’t need to create something brand new, but instead can look at interventions that have been successful in other similar areas or settings. So, let’s progress through this assignment one step at a time:

  1. 1. Learn about Social Determinants of Health (SDoH)

“The social determinants of health are the conditions in which people are born, grow, live, work and age. These circumstances are shaped by the distribution of money, power and resources at global, national and local levels. The social determinants of health are mostly responsible for health inequities – the unfair and avoidable differences in health status seen within and between countries” (WHO, 2019; para. 1) (Links to an external site.).

  1. Create a PICO question based on your selected intervention. Although exact wording of your PICO may evolve throughout the next few weeks, this question will become the focus for the remainder of your assignments in this class, so it is important to craft this question well. The following resources will help you create a solid PICO question:

      1. Indicate the specific SDoH that you selected, including a specific angle that you would address
      2. Identify the specific PICO components on which you will focus.
        • Population: indicate which community-based population will receive the intervention
        • Intervention: include the specific intervention you feel could address SDoH in your community
        • Comparison: select a comparison to the intervention (often how things are currently done)
        • Outcome: indicate how you would determine effectiveness of the intervention (measurable)
      3. Create and label a single PICO question (1 sentence, ending with a question mark).
      4. Identify the potential implication(s) of the finding towards improving health sustainability (1 paragraph).

  1. Social Determinant of Health (issue): Education (school attendance)
  2. Components:
      • Population: elementary school children in low-income schools
      • Intervention: leader for “walking school bus” each morning and afternoon
      • Comparison: unsupervised walking to school
      • Outcome: increased attendance rate
  1. PICO Question: For elementary school students in low-income schools (P), are attendance rates better (O) when a “walking school bus” (I) is done daily compared to when students walk unsupervised to school (C)?
  2. Implications for Health Sustainability:
    Regular attendance could benefit students three ways. First, this allows students to have more lessons about health and how to care for their bodies. Second, regular attendance increases access to healthy meals each day, particularly for low-income families/schools that qualify for a free breakfast and lunch program. Finally, increased attendance can promote social interactions such as friendships and partnerships, which can be beneficial for mental health. Therefore, implementing a “walking school bus” for low-income schools could help students improve their health knowledge, diet, and sociability, potentially providing sustainable physical and mental health benefits.
