Overview

This discussion asks you look at the role of the family health nurse in a genetic counseling scenario. Imagine that you are a family health nurse, and today in your office you will be seeing a married couple who are both carriers for a genetic disorder (for example, sickle cell, cystic fibrosis, or Huntington’s disease). This couple would like to have children, but since they are both carriers for a genetic disorder, they are seeking your advice on what they should do before they attempt to start a family.

Your initial post must be posted before you can view and respond to colleagues, must contain minimum of two (2) references, in addition to examples from your personal experiences to augment the topic. The goal is to make your post interesting and engaging so others will want to read/respond to it. Synthesize and summarize from your resources in order to avoid the use of direct quotes, which can often be dry and boring. No direct quotes are allowed in the discussion board posts.

Post a thoughtful response to at least two (2) other colleagues’ initial postings. DB 2 Genetic counseling

Preconception counseling is ideal for families considering pregnancy to gather information, reduce risks and discuss genetic decisions without the time constraints of a pregnancy (Kaakinen, 2018). In this discussion board, I will play the role of a genetic health nurse dealing with a married couple that are both carriers for Cystic Fibrosis (CF).

According to the American College of Obstetrics and Gynecology (2017), since both of you are carriers there is a 25% chance that your baby will have CF, that is a one in four chance. There is also a chance that your baby will not have the disease but will be a carrier like you. Finally there is also a 25% chance that your baby will not be a carrier and will not have CF. I know this is a lot to take in and we have a staff member with a child that has CF that would be happy to discuss her experience with you now, as well as current treatment for the disease.

With this information, you have many options. If you are comfortable with the 25% risk you can choose to get pregnant. When you become pregnant there are tests such as an amniocentesis that will tell you if the baby has CF. If that is the case you can choose to continue with the pregnancy or terminate.

If you are not comfortable with the 25% risk you could adopt or have a donor sperm or donor egg of a non-CF carrier because both parents have to be carriers for the baby to get CF (American College of Obstetrics and Gynecology, 2017).

In conclusion, genetic nurses are responsible for educating families on genetic issues and medical options available for their care.

This is my initial post all i need with this question is a response from the class mate i sent.

 

In this scenario, being a family nurse, I would assume the position of both the case manager and a health educator. In the position of a case manager, I would evaluate the case from a medical point of view, which will encompass getting a detailed assessment of the couple’s history and that of their close family members (Kaakinen, Coehlo, Steele, & Robinson, 2018). That will help in the process of evaluating the prevalence of genetic disorders in the family. Additionally, I will look at tests that had been conducted on the couple and interpret them in the context of what it means for the couple’s dream of having kids. After that, I will analyze whether any risk factors predispose the couple to have kids who have genetic disorders. In a general sense, my role as a case manager will be restricted to presenting all of the viable options to the couple.

As well, I will also play the role of a health educator for the couple.That will be in the form of advising them on the way forward (Kemppainen, Tossavainen, & Turunen, 2012). There is a huge possibility that the tests might not be favorable to the dreams of the couple to have kids. That being the case, it can crush their spirit. At that point, I will step in to educate and counsel them on the way forward. To give an example, I might connect them with some of the best adoption agencies. In that way, they will still get to have kids even though it is not in a conventional manner. On the other hand, if there is something that can be done medically, I will advise them in the best options they should take to ensure a good pregnancy and hence a healthy baby.
