    Scenario

You work for a company that performs comprehensive risk assessments on patients with Medicare plans. The visits are conducted in both skilled nursing facilities and in-patient homes. The purpose of these comprehensive assessments is to gain a clearer picture of the patient’s health status and chronic disease management. After completing an assessment, you may collaborate with the patient’s primary care provider or refer the client for additional health services. The clinical manager calls patients after visits to elicit feedback about their experience. General feedback—not specific to any single nurse—includes the following:

    • “The nurse seemed rushed. He did not close the door to my room before beginning the exam, and I did not feel comfortable during the visit.”
    • “The nurse seemed frustrated that I wanted my husband and daughter to stay for the exam. In my culture, we make healthcare decisions as a family.”
    • “The nurse just tapped on my abdomen. I don’t know what that would tell her about my stomach pain.”

The manager asks for your help with staff training to address these concerns. You plan to develop a paper-based resource to improve nurses’ advanced physical assessment techniques, clinical reasoning, and judgment.

    Instructions

Design a paper-based training resource for the nurses in the scenario. The resource should be in the form of an aesthetically-pleasing handbook, including properly cited images where applicable. The training content must cover the following:

    • How to ensure privacy, comfort, safety, and patient understanding throughout the advanced physical assessment.
    • Ways to integrate cultural considerations into the advanced physical assessment.
    • Descriptions of inspection, auscultation, percussion, and palpation, including:
    • Use of each technique as part of an advanced, comprehensive physical assessment.
    • How to explain the techniques to patients during the exam.
    • Examples of normal and abnormal findings demonstrating clinical reasoning and judgment.
