Documentation of the processes that nurses use for delivering patient care is a fundamental tool for communicating the current health status of the patient to the clinical team and provides evidence during incidents with legal and ethical implications. In this case, the failure of the admitting nurse to document that assessment of the patient played a prominent role in the decision of the court regarding the complications that resulted in the death of the patient. According to Duclos-Miller (2016), accurate documentation is the only proof that nurses can use to show their credibility that their actions were consistent with the professional standards for ensuring safe and quality patient care.
