Pediatric Disorders of Eye, Ears, and Oral Cavity

Last week at my clinical we had a 15-month-old boy with the mother complaining that her child’s eye has had some swelling and tenderness to the left eye. There has also been crusting to the eyelid, according to the mother. On examination, there was a pus-filled bump at the edge of the bottom of the lower eyelid. The diagnosis for the patient was Hordeolum stye. According to Johns Hopkins Medicine (2022), a hordeolum stye is a red bump on the edge of the eyelid. It occurs when the gland on the eyelid is infected, usually caused by the bacteria called staphylococcus aureus (Yang et al, 2022). It can be classified as either internal or external (Sun, 2019). In this case, the stye was classified as external. Symptoms may include redness, tenderness, swelling, and foreign body sensation in the eye. There may also be some tearing and crusting to the affected eye. Sun (2019) states that a rare complication may include orbital cellulitis, which can lead to sight-threatening and even life-threatening complications. Differentiating this includes examination of visual acuity, pupils, extraocular movements, and eyelids. With supervision from my preceptor, we recommended warm compresses (Sun, 2019) to the left eyelid for five to ten minutes with ofloxacin (Yang et al., 2022) eye drops one drop to the left eye three times a day for seven days. Teaching included hand hygiene for patient and caregiver of the patient along with reminding the caregivers to never try to squeeze the stye. The patient’s caregivers verbalized understanding of both the treatment and education given. They were also instructed to return to the clinic within two weeks.
