- Perform a direct assessment of a community of interest using the “Functional Health Patterns Community Assessment Guide.”
- Interview a community health and c provider(example a charge nurse) regarding that person’s role and experiences within the community.
Interview Guidelines
Interviews can take place in-person, by phone, or by Skype.
Develop interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.
Complete the “Provider Interview Acknowledgement Form” prior to conducting the interview. Submit this document separately in its respective drop box.
Compile key findings from the interview, including the interview questions used, and submit these with the presentation.
PowerPoint Presentation
Create a PowerPoint presentation of 15 slides (slide count does not include title and references slide) describing the chosen community interest.
Include the following in your presentation:
- Description of community and community boundaries: the people and the geographic, geopolitical, financial, educational level; ethnic and phenomenological features of the community, as well as types of social interactions; common goals and interests; and barriers, and challenges, including any identified social determinates of health.
- Summary of community assessment: (a) funding sources and (b) partnerships.
- Summary of interview with community health/public health provider.
- Identification of an issue that is lacking or an opportunity for health promotion.
- A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.
While APA style is not required for the body of this assignment, solid academic writing is expected, and documentation of sources should be presented using APA format ting guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

This week you will be completing a community assessment. This is a physical community such Anytown, USA and not a community such as veterans or African Americans. It would be best to use your own town/city for this assessment. You might have heard the Term Windshield Survey, this is what this assignment is. It would as if you were driving around the community and looking at the make-up of the city and what resources are available. Are there parks available? Places for kids to play? A community center? A lot of churches? A hospital or just clinics or nothing at all? Make sure that you are looking at what vulnerable populations are present in this community? Is it refugees, the homeless, low-income families, non-English speaking community members? When doing this assessment, you need to look to see what community-based resources are available for the community. Think back to Week 1, when you talked about different community-based resources. These can include churches, community centers, clinics/hospitals, and even schools within the community. As you are looking at these resources, look at how these resources can advocate for the vulnerable groups you have determined are in this community. Use the Functional Health Patterns as a guide to what should be included in the presentation.
When working on your Community Assessment and Analysis presentation, we have changed the requirements for the Interview. Typically, you would have had to interview a public health nurse for your community but that is not feasible at this point. Instead, I would like you to interview another community healthcare worker. This can be a charge nurse at the hospital or another healthcare worker who is familiar with the health issues the community is dealing with at this time. You are wanting to know what health issues the community is experiencing. If there is a pollution in the community, then maybe there is an increased amount of asthma or other respiratory issues prevalent. It may be diabetes, or heart disease. Let me know if you are having problems with this part of the assignment. You should include a summary of the interview on a slide within the power point presentation. Make sure that you look at the rubric for this assignment so that you are including all items required. A conclusion slide is required.
In the Community Assessment and Analysis presentation, make sure that you are including pictures/graphics in the power point. This is part of the grading and makes the power point more interesting. Also, when using a picture, you always need to include a citation for the picture. In the Notes section at the bottom of the power point page, you just need to include the web address of where you obtained the picture/graphic. You do not need to include these references in the references that you have at the end of the power point.
For the Provider Interview Acknowledgement Form, if you talk with the nurse/healthcare provider via the phone, in the area where they should sign the form, just type in their name, and state that you spoke with them on the phone and include their phone number.
