Scenario:

It is National Stroke Awareness Month, and as the nurse educator at a local hospital, you are asked to provide a conference for healthcare professionals to advance their knowledge and skills regarding cerebrovascular accidents. You choose to use the following case study as your teaching method:

Mike is a 66-year-old Hispanic male who presents to the emergency room at his local hospital with acute aphasia, right facial droop, and right-sided weakness. BP is 168/110. The sudden onset of symptoms occurred at the post office, where he works part-time. Upon Mike’s arrival at the hospital, the ER nurse proceeds to gather the patient’s medical history from his wife, Isabella, who accompanied him in the ambulance. She tells the nurse that Mike has a history of uncontrolled hypertension, and he was often non-compliant with his anti-hypertensive medications. His recent diagnosis of diabetes was noted as well as the oral hypoglycemic agents he was taking.

The results of laboratory tests, including a complete blood count, prothrombin time, serum electrolyte levels, cardiac biomarkers, and renal function studies, are all within normal limits. CT of the brain indicates a thrombus in a branch of the right internal carotid artery, with approximately 50% occlusion due to atherosclerosis. There is an area of infarction in the right anterior hemisphere. There is no evidence of a subarachnoid hemorrhage. The diagnosis is made 2 hours after the patient’s arrival in the emergency department. Mike is then treated with thrombolytic therapy and will be placed in the hospital’s intensive care unit for further monitoring of his condition.
